In answer to your question, yes, I believe it happens everywhere. But something to think about is some of those cars also need those spaces. The van spaces are, generally, the only spaces that have hash marks for no parking next to them. For me, I need that space to fully open my door so I can unload my manual wheelchair. The other handicap spaces without those hash marks mean that a car parks too close to my door for me to load and unload. Do individuals without wheelchairs often park in those spaces anyway? yes. And it is frustrating.
I have a friend who carries cones in the van with her. And if parking is a problem, she parks over a centerline then puts the cones out while they are in the store or wherever they are. It's not ideal. It shouldn't be that way. But sometimes we improvise.
